The perspectives of physicians regarding antidiabetic therapy de-intensification and factors affecting their treatment choices- A cross-sectional study
Objective: Comprehensive diabetes management may include intensification of treatment or a combination of antidiabetic therapy. However, this may be associated with an increased risk of side effects and death. The purpose of this study is to understand the perspective of physicians regarding the treatment of de-intensification, HbA1c goal of individualization, and the factors that influence their treatment options for patients with type 2 diabetes mellitus (T2DM).
Methods: Cross-sectional study carried out in primary and secondary care unit in Saudi Arabia using an online questionnaire. Two previously validated questionnaire used to understand consciousness, deal with, and their practices of individualising goal HbA1c and optimization of antidiabetic treatment, and to assess the factors that influence physician treatment options physicians when prescribing oral antidiabetic medication for patients with diabetes mellitus type 2. The study population is a doctor who treats patients with diabetes mellitus during the period between October 2017 and May 2018.
Results: A total of 205 physicians participated in the study. Approximately 50% of physicians have a special family medicine (n = 98, 47.8%). A majority of physicians (n = 183, 89.3%) are familiar with the concept of individualization goal HbA1c. However, only 66.3% of them (n = 136) reported that they apply it either always or most of the time. 58.5% (n = 120) of physicians reported that they would not start a conversation about de-intensify antidiabetic therapy even if their patients had stable HbA1c value during the year. The doctor showed a higher consideration for objective patient clinical data and their assessment of the patient’s health status, with a minor consideration factors related to the patient.
Conclusion: Health professionals should be more focus on the implementation of contemporary practice and apply the necessary treatment de-intensification or dose adjustment. subjective patient factors should be considered further, because these factors are associated with better disease management.
The decline in cross-border tobacco purchases despite the intensification of antitobacco policies in France
Background: Recently, France has intensified tobacco control policies including a gradual increase in the price of tobacco products and the introduction of plain packaging. However, it has been no suggestion that cross-border purchase of tobacco from neighboring countries, with lower tobacco prices, will increase. We examined trends in cross-border purchase of tobacco among smokers in conjunction with the implementation of tobacco control measures between 2016 and 2017.
Methods: Description des Perception, Image, et Comportements located au Tabagisme are two waves of cross-sectional national telephone survey of adults 18-64 years old French, which recruited a total of 2167 smokers (2016: n = 1238; 2017: n = 929) , Data were weighted to be representative of the adult population of France. The relationship between the waves and the purchase of tobacco studies examined through a wave of transnational studies using multivariable logistic regression models (adjusted OR: ORa (95% CI)).
Results: Less than half (38.5%) of smokers stated cross-border purchase of tobacco in the past year, mostly carried out regularly: 22.6% cross-border tobacco purchased once or twice a year. In 2017, compared to 2016, cross-border purchases of tobacco by smokers France declined (ORa = 0.81, 95% CI 0.68 to 0.98). Other factors associated with cross-border tobacco purchases included sex, and within driving distance to the border.
